The following are links to commonly used forms and /or court documents. If you have any suggestions for other forms that should be offered, please email us and let us know. Driver Safety Course Packet for individuals who have already requested a driver safety course for citation dismissal and weren't able to print the documents on the payment website.  You may print this packet to submit to the court.  DSC Packet.

Citizens can file complaints with the court for the prosecutor to refile for filing a case with the court. Please print out the Citizen Affidavit Packet, fill out completely and sign in front of a notary or in front of our court clerk. This is for NON-TRAFFIC offenses.
